Service Agreement Terms and Conditions

I am hiring you (Medical Licensure Group, LLC) to assist me in applying for state medical licensure, registrations and credentials as specified in our Service Agreement, as selected during the online signup process or as requested either verbally or in writing by myself, my employer or representatives. I agree to provide you with all information and documents required for you to perform the services, whether directly or via my employer or representatives. I affirm such information is and will be accurate and complete and such documents will be true, correct and complete copies. I acknowledge that any information or documentation provided to MLG or obtained by MLG may be shared with my employer or its designated representatives during the licensure process unless otherwise requested in writing. I am aware of, and remain responsible for, the eligibility requirements and deadlines for each license, registration or credential for which I am applying with your assistance. I agree to pay all application and credential verification fees or other charges required or imposed by the licensing boards and/or credential holders, as invoiced by you, to file or process my applications for licensure, registration or credentials that are the subject of the services. I acknowledge you have no authority to grant or cause to be granted any licensure, registration or credential and, therefore, you will not be liable to me or any other person regarding the outcome of my applications for licensure, registration or credentials. I further acknowledge that you make no guarantee of timelines in which any licensure, registration or credential will be granted. I acknowledge I have read and agree to the additional terms and conditions below.

Flat Rate Policy

I acknowledge you use a streamlined system that enables you to perform the services effectively. Typically, your services include only application preparation, credentials verification requests, and follow-up and management of the application process until the license or credential is granted. If I require assistance that goes beyond the typical services scenario (e.g. – extensive malpractice / adverse action documentation, lengthy credentials history, etc.) you may bill me additional amounts for those untypical services. You will notify me in advance, however, of any such untypical services that would result in me being billed additional amounts.

Third-Party Fees

I understand and acknowledge that the service fees paid are exclusive of the state board fees and the fees charged by my credentials holders to process requests for direct-source verification of my credentials. I further acknowledge that I will be invoiced for the fees and that payment is required to complete the licensing project on my behalf.

International Credentials

I acknowledge your services will not include collecting records held or maintained by persons that are located outside the United States of America. If any licensing board requires records from persons that are located outside the United States of America, I will obtain those records.

Project Timeline & Client Responsibility

Licensing & Credentialing Projects:

I acknowledge that the focus of MLG is to ensure timely processing of my licensing and credentialing projects. To ensure momentum is maintained, I agree that if my project is stalled for a period exceeding three (3) months due to lack of response and/or missing information on my end, it will be considered abandoned, and the project closed without further follow-up efforts. Reactivation will require a new agreement and fees to restart services.

Maintenance Projects:

I acknowledge that your service fees must be paid in full prior to maintenance services such as license renewal and data maintenance being initiated. I further acknowledge that MLG will make all reasonable efforts to provide advanced notice of any upcoming credentials scheduled to expire. Should my credentials not renew in time due to lack of communication and/or non-payment on my part, I will assume full responsibility for the status of my license/credential and its non-renewal. I acknowledge that inquiries and attempts to obtain information and/or approval must be responded to well in advance of the expiration date and during normal business hours, not including weekends and federal holidays.

Refund Policy

Licensing and Credentialing Services:

I acknowledge that MLG aims to deliver high-quality, hands-on licensure and credentialing services that begin promptly upon signup. Because of the specialized nature of the work, I understand that internal resources are allocated immediately, and progress is typically well underway within the first week.
Refund Eligibility:

  • Within 24 hours of purchase: 100% refund
  • Within 7 days of Purchase: Up to 50% refund
  • Within 30 days: Up to 25% refund
  • After 30 days: No refund available.

Requests for refunds must be made in writing.

Subscription-based Maintenance & Monitoring Services:

Clients may cancel their maintenance or monitoring subscription at any time. However, all payments for the current billing period are non-refundable, as ongoing resources and support are allocated in advance. Services will remain active through the end of the paid term unless otherwise agreed in writing.
Requests for refunds must be made in writing.

Professional Conduct

I acknowledge that I am expected to maintain a professional and respectful demeanor in all communications and interactions with MLG’s team. Disparaging, disrespectful, or inappropriate behavior toward staff or representatives will not be tolerated and may result in terminations of services. I acknowledge that the foregoing refund policy will apply to such circumstances.
